I've been a VPBX customer since 2006 and it has served my SMB business needs very well, with virtually zero problems. Priced right, full-featured, my only desire to to have them do voice-text voicemail transcription, but otherwise a stellar service.
I don't use outcalling through the PBX, and largely this just handles inbound calls, but does it reliably and professionally.
