If I had a couple of convictions 35+ years ago and can’t remember what they were, how do I complete the SBA 912 form? I ran a simple background check it it shows case numbers but Undisclosed. I think one was for giving a false name while drunk in public and the other was for a petty theft…beer run gone bad.

The general rule given to us by the SBA is that any conviction in the last 6 months must be disclosed. However, if you’ve ever received a felony, at any point in your life, then they must be disclosed. If you’re unsure whether or not you had a felony then I would contact the county records in the place the conviction took place to find the information on it. You don’t want to not-disclose a felony and then have the SBA find it in their background check. That would greatly diminish your chances of being approved.
